Item 1.01. Entry into a Material Definitive Agreement.
FormFactor, Inc., a Delaware corporation, entered into Amendment “V” to the Intel Corporation Purchase Agreement — Capital Equipment and Services dated January 8, 2001 by and between FormFactor and Intel Corporation (as amended from time to time, the “Purchase Agreement”). Amendment “V”, which has an effective date of January 8, 2005, extends the term of the Purchase Agreement through July 8, 2005. FormFactor entered into Amendment “V” on January 3, 2005.
